Health Coverage Tax Documents

Request a Copy of Your Form 1095-C

Individuals who were enrolled in coverage during the applicable calendar year may request a copy of their Form 1095-C (Employer-Provided Health Insurance Offer & Coverage) by emailing 1095C@fairfieldschools.org. Please include your full legal name and date of birth in your request. A copy will be provided within 30 days of the date your request is received.

Accessing Your Form 1095-C Online

Employees may also access their Form 1095-C through the Employee Self Service (ESS) portal:

After logging in, select Pay/Tax Information and choose 1095-C to view or print your form.
