Insurance/Benefit Enrollment
Welcome to Fairfield Public Schools
Our benefit enrollment module is online through Employee Self Service (ESS). You can access it here: Open Employee Self Service (ESS)
You will receive an email from the FPS Insurance Department indicating that you can now enroll in insurance and benefits using the Employee Self Service module. Please do not attempt to log into the Employee Self Service module until you receive the email from the Insurance Department.
Logging In
- Username: first initial of your first name, your last name, and the last 4 numbers of your social security number (no spaces).
- Temporary password: last 4 numbers of your social security number.
If you have difficulty logging into the ESS module, email HRESS@fairfieldschools.org for assistance.
Once you have logged into the Employee Self Service module, select Benefits from the right-hand menu to make your elections.
Required Documentation
- If you are enrolling a spouse, we will need a copy of your town issued marriage certificate.
- If you are enrolling children, we will need copies of their birth certificates.
- Social security numbers will need to be entered for any dependents that are being enrolled.
If you are electing Life Insurance and/or Disability, please complete the Lincoln Financial Group Enrollment form and Beneficiary form (if electing Life Insurance). These can be printed from the enrollment module. They are also located on the FPS website under Departments → Faculty & Staff → Benefits → Forms.
Returning Your Documents
Please return all documents and forms using one of the methods below:

Additional Information
